Quillboar in starting area, was doing fine until 2-3 mobs respawned near my druid and yeah, dead. XD
At least it wasn’t murlocs on that isle in Elwyn Forest for once. 
I don’t know. I think it was the ogre like creatures in Teldrassil. Perhaps in the cave or maybe outside.
I’ve always found pushing the limits to achieve optimal efficiency fun in Vanilla. But then when you die it’s a set back. Every move you make is a gamble. Will I win time if I just run through and try to shake off the 3 mobs or will I die and lose time.
Not that time is such an issue.
I was killed by 2 scarlet’s in tristfall. They were 2 levels above me xD
npc resisted 2 big spells and I ran out of mana and died (:

The Scarlet dude with the 2 bodyguard. I blindy attacked his guards, who were level 8 and only then saw that the dude himself was level 12. I was level 9 at the moment - and a squishy priest who went OOM in no time.

To a prowler in Elwynn Forest. I had a tiny bit of health left, the Prowler had a tiny bit left, I didn’t want to use my last health potion so I just kept fighting and hoped I would get the killing blow in before the Prowler did. Alas, I died. My only death so far actually, though a harvest watcher came close earlier today.
I forgot breathing, so I was swimming in Durotar and then… EWWW AWWWW

Face pulled another murloc for the kelp quest in Elwynn, it was GG at that point.
Don’t worry, I’m expecting absolutely loads when my druid gets near to Sludge Fen when it comes to quests around there. Especially the swim form questline. That’s going to be interesting, that’s for sure doing that all over again.
Not my first but definitely the most memorable 5 deaths due to dark iron dwarves in logh modan… I needed the ore from this guy for my warrior quests and the guys were just too strong for me or spawned at an unfortunate time.
The graveyard was like a 10 minute walk! Noted to self that potions would’ve come in handy.
Went AFK and even though i left my character on a hill behind a tree, when i returned i was dead.
I was killed by the Que.
I fell down a hill 
My first and only death so far was north of Dolanar adding two more of those spiders who just respawned while I was fighting another one farming for the cooking quest.
I died to a voodoo troll on the echo isles. She spawned right after i killed the hexxed one.
Got her down to low hp and she healed herself which then i begun running around the hill 5 times. SHE. WAS. STILL. CHASING. ME.
There was a buzzing insect in my room. I went to investigate, and when I came back a Tirisfal farmer spawned on me and had beaten me to a pulp.
