I’ve done Withered Army Training several times and am Exalted with the Nightfallen. As I’ve got quite a lot of spare Ancient Mana I thought I’d do another training run or two just so I can get another Nightfallen Cache.
But Thalyssra’s nowhere to be found, so I can’t reacquire the quest.
Does she disappear once I reach Exalted? If not, where is she?

Unless this changed (I don’t think it did with SL) and if I remember this correctly, once you did the intro scenario to Withered Army Training it turns into a world quest that can only be completed once every 3 days, and since Thalyssra is the turn-in for that world quest, she won’t show up in the scenario unless you wait for the cooldown to reset.
I’m not sure if you can still get other rewards by doing it more than once every 3 days (I seem to remember you can’t), but hopefully the above at least helps to solve the mystery around Thalyssra 
I’ve found her! Inside the cave, on the lower level. I can acquire the quest from her, even tho’ she doesn’t have a ‘?’ above her and she doesn’t show on the minimap.
Just to add to what Kendell said, Thalyssra roams around the cave, and is often in different places across the upper and lower levels of the cave =)
